Ringer Vibrate turns off on reboot. Any ideas why?

UPDATE: If phones in vibrate mode, then I shutdown and restart it, the ringer turns back on. If I have the screen on and hold in the power to tap the vibrate icon, it takes 2 attempts to get it to toggle vibrate on. The first attempt the vibrate icon shows up in the menu bar up top, but then dissappers. The 2nd attempt puts it back in vibrate. It didnt always do this, but perhaps an app could cause this? Maybe its fighting for some type of volume privilege? Any ideas?
