Samsung Paying for your phone until 2016 question? [Note5]

so the new Samsung promotions came out for the note 5 if you switch from iPhone. So my question is to anyone who has done the Samsung promotions, what do you need to complete it? I am switching from an iPhone. My iPhone just broke so i said hey i will switch to note 5 do i need to turn my phone into t-mobile to get a receipt or something that says i'm switching from iPhone to Samsung or what is needed does anyone know?

Oops just read the part where you said your iPhone was broke. Your t-mobile rep would probably give you the best answer.
