I understand much about Android. But one thing I still don't understand is the technology that during a call the person could pick up nearby sounds. Yet if I play full volume a video or music from the phone I'm calling the other person can't hear it. Even on speakerphone. Why is that? Same way around as well.

Welcome to Android Central! I'm curious, which app are using for video or music? When I make a call while using Google Play Music or YouTube, the playback automatically pauses.

When you're speaking while the music is still playing, do you notice the volume of the music drop temporarily?
