SD Card file structer won't change

So.... I've never seen this before and not sure if it's an Android/ROM issue of unable to update an index of files or if the SD card is pretty much dead....

I download, move, create any file on my SD card, from the phone itself or from the PC and for the moment the change looks to have been made. I unmount the sd card from the PC or reboot the phone, it "undos" whatever I did (move files, new files created, etc).

Want the real kicker? I've formatted this memory card through the Android OS, through Xionia recovery, and through Windows. All said it was successful, but when my phone reads it, the files and folders are still there. Now, I haven't tried using any of these files yet (at work, can't play much) nor am I currently able to remove the sd card and try it elsewhere yet.

I'm thinking formatting system, data, and the sd card could solve this problem, but I'm afraid I won't be able to get a ROM back to the sd card and I'll be s.o.l. all day (yes, I have a spare card at home so it's not a total loss).

What could this problem be?

EDIT: So far, the photos taken with the camera appear to work after formatting - so the card WON"T format

SD Formatter 3.0 for SD/SDHC/SDXC - SD Association

You can try that utility and completely wipe the card.

Then use Windows XP / Vista / 7 to reformat the card as Fat-32

You might also be able to download GPARTED as an ISO, burn it, boot with it,
and use that to delete and re-create the partition table and format as fat-32 as well.
 
Try deleting the partition table itself and then recreating it. I'd say that or the sdcard might be write-protected in some weird way.
 
Eh, I think the card is dead. Just got home, plugged the card into the SD adapter, tried backing it up... error, unable to copy files from it. Tried formatting through Windows, unable to format. Trying that program DoesItMatter suggested in a second... but I'm not too hopeful anymore... Sandisk has lifetime warranties right?

I'm thinking the card is dead. I used this tool to try to format, it said the card was locked. IF it in fact is locked, I don't know how I did it. The lock tab on the card (or adapter I should say) is unlocked. If anyone has any other ideas, great. Otherwise this is an over-priced 16GB card that might get tossed here really soon.
