sd card help

so ive been rooted on my bolt for almost a year since i got it ... never really had a problem with it running liquid smooth 3.2 i believe .... a couple of months ago i kept having to reboot my phone to get access to my sd card, be it pictures or music or files .... i went and re-formatted my sd card and all was fixed .... now its happening again so i just reformatted and wiped my sd card and then put it back into my phone and added back 3.2gb of files from a previous backup of my sd card and when i go to use my camera it still says "please insert sd card to use camera" any help would be much appreciated

Have you tried unmounting and then mounting the sd card through settings/storage? Also, which ROM are you using? I know that some 3.5 ROMs have issues with the sd card.
