Have you tried the battery pull for 5 mins. to clear the cache?
My only problem is that my contact ringtones get messed up and I have to reassign them....no biggie for me, I only have family members and close friends (20) that get custom ones!
Actually, as I remember it, there were numerous complaints of this even in Froyo.From what I've noticed the ringtone plays one behind. ie custom 1 calls it may or may not be right, then default calls and it plays custom1, then custom3 calls and default plays, custom4 calls and custom3 plays, and so on. The only solution I've found comes via rooting and installing your custom tones into your phone and changing the permissions. I've been looking for a fix for this for some time to no avail. Just another SNAFU from the gingerbread update that got through. I'm sure it is in the base because I've heard of people even using the stock ringtone for custom ringtones and it still messes up. The rooted people have had this problem for awhile now the OTA people get to reap the benefits of it.

I rerooted my phone last night, and tried fatboy97's suggestion about moving the ringtones to the phone's root and changing the permissions...and they are still doing the same as before....I did notice that if my phone hadn't rang in awhile, when a call comes through it would ring with the correct ringtone...not sure of the lapse in time before calls though...baah humbug....Just because I was curious I called Verizon tech support to see what they would suggest...the level two tech suggested I download an app "mortring" that assigns custom ringtones to contacts....didn't work...then he suggested that he send me a replacement LG Revolution...lol....I would rather go back to a dumb phone before I use a Revolution, I heard some pretty bad things about that one...
