T-Mobile HTC One - Name ID doesn't work on my phone

I recently purchased an HTC One for another line on my account. I noticed when making calls it would show the name, city, and state, and I eventually linked it to the T-Mo Name ID app. I declined the trial but it still shows the state that the call is coming from, which I like. However, my HTC One doesn't do that. It appears Name ID will still provide some functionality even after declining the trial (I tried disabling it on the original HTC One and it doesn't show the state; when re-enabled it does). On my One, it simply says it "is unable to contact the server" immediately after opening the app. I tried disabling and re-enabling and also clearing the data for the app.

Has anyone had this issue and cared enough to look into it?
