This screen is KILLING me

[AC Question]

I have a note 4. And somehow I downloaded a program that has a swipe screen. Even after I use my finger print to open the phone I have to swipe this one. HOWEVER sometimes it dont swipe all the way so I have to restart the phone. How can I find out where it came from and get rid of it. I was going to post a pic of it but cant add it..

Please help me remove the swipe!!

There is a swipe screen that is on my phone. I must have got it from an app I downloaded. I have done everything but Completely formatted my phone to try and get rid of it. It wouldnt bother me all sept sometimes it wont swipe to unlock and when it dont I have to restart the phone. I would greatly appreciate any help as to how to get his annoyance off my phone!!!
