tough time pairing w/a PLT m50 BT HS

The pairing won't stick and it's never letting me enter a security code

PLTm50 shows up as available, on the GNEX BT setup, I press it, I get a "pairing successful", the PLT m50 shows up in "paired devices:", then drops down to the "available devices again, I try to pair and get a "couldnt pair with...because of an incorrect PIN or passkey". yet it never asked me for one

It's like an endless loop. I can't seem to just delete the pltm50 from BT list and start fresh. Reboot gnex no help

any ideas? Never had this w/all the Moto BT headsets I have paired.

i think the headset is a dud. I plug it in to charge, the red light only goes on for 3 seconds, goes off and won't charge past medium
