Unable to apply themes

I like to switch things up on my Epic and change themes fairly often. Recently I have noticed that themes either do not get applied or only pieces of them get applied.

For example, there are the steps I am taking:

1. Boot into ClockworkMod Recovery
2. Wipe cache partition 3x
3. Wipe Dalvik cache 3x
4. Install zip from SD card and choose appropriate zip, which in this case is Shake and Bake Mix-Up theme
5. Reboot my Phone

When my phone boots back up, everything remains the same as the previous theme I applied. If the theme modifies items that were not previously modified they are changed but otherwise the rest of the theme stays the same.

Any suggestions?

If you're on an MTD rom, mount /system and (this is optional, but it works for me) /data, then flash.
