Unresponisve Screen

Has anyone else noticed that their phones screens are somewhat unresponsive. Sometimes when I tap an app or a button while playing a game nothing seems to happen. Its not that it lags its like it doesn't register that i tapped the phone at all. I have a verizon version and this has happened under normal mode and while having the "increase touch sensitivity" selected.

This happens to mine once a day. Also when the screen turns off during a call using the proximity sensor, it won't turn back on when the phone is pulled away from my face

I've noticed it a bunch of times. Never had an issue with it on my note 2.
