Vibrate mode turning off by itself

I'm not sure what's going on, but when I turn the volume down on the ringer and put the phone in vibrate mode, after a while the volume goes up again. I have no idea why vibrate mode is disabling by itself. Any help would be appreciated. I'm completely stock, through the March update. Thank you.
 
Screen is being activated in your pocket and volume buttons being pressed? That's one thing I don't like, that you can change the volume while the phone is locked.
 
This happens to me too, sort of. I set up do not disturb "until I turn it off", head into a meeting, set the phone down, and (only sometimes) after awhile, my phone will make noise. It seems to turn itself off. I do not allow the screen to turn on for notifications or when you pick it up off a table.

Joe, I thought that at first, but it also happens when I'm using the phone. I'll be reading something and all of a sudden I'll realize that vibrate mode is no longer in effect. And I know I haven't pressed the volume keys and the volume slider never came up. I unfortunately, can't reproduce it reliably. I do know, however, that the volume always gets set to the lowest audible setting.

For the time being, I just have a tasker profile set up to turn the volume to 0, if volume level ever equal 1. It's a stupid hack and it means I can never have volume level set to 1, even manually, but I'm OK with that for the time being.
