Volume doesn't work for apps?

I have a nexus 4 and it's been acting up recently. Things like text tones and calls will go through but no sound from apps will play? I also noticed that google voice pops up now whenever I plug in my headphones?

Make sure you turn the volume up when a game is running - you have one set of buttons to control 4 different levels - ringtone, notification, alarms and media. The buttons change the volume of whatever is running when you press them. (The default, with nothing running, is ringtone.)
