Volume Problem: Glitch or Safety Setting?

On numerous but not all occasions, when I plug my phone (Droid Maxx) into a car stereo jack to use it as a MP3 player it mutes itself. This is not like a normal mute though, I try to turn the volume up with the side volume rocker, and I go into Settings under Sound - Volumes and it will not allow me to adjust the volume there either. The speaker display on the Volume settings shows to be shadowed out with a slash through the speaker. I have to reboot my phone to gain access to the volume control, then it usually works in the car. On a side note one of my cars has a bluetooth compatible stereo but it tends to fade in and out almost like an edited song when I attempt to play my songs through bluetooth. Not sure if it is the device or the stereo in that instance. Has anyone else had this self muting problem? I appreciate any input.

Sounds to me like the audio driver is crashing or getting stuck as a result of switching modes. I have not had that occur though.
