My speaker phone and volume when watching videos is almost nonexistent. In have the volume maxed out. Any solutions? 5.1 m build.

First, turning the volume up when you're not watching a video turns the ringtone volume up. You have to turn it up when you're listening to music or watching a video (the media volume).

Second, it could be a defective phone.
 
I know that. It was working just fine the day the new star wars and jurrasic world trailers came out. All of a sudden, it stopped.

That is so annoying! We need to be able to adjust alert or media volume at any time, regardless what the phone is doing at the moment.

In 5.1 they added the ability to switch from the media volume slider over to the alert panel, but not the opposite.

Short of a factory reset, which may or may not fix the problem, what can/should I do to fix this issues?

Is this just for videos? Try playing music. Also try MX player if you downloaded the video. It could be a missing codec, MX has amazing codec support.

Volume sucks on this phone. It's basically only audible between 90%-100%, and really only functionally audible at 95%-100%.
