Well a Nandroid Backup makes a complete copy of your systems image. So when restored it will be as it was when you did your backup.

Titanium and Mybackup Root only do data/settings/apps backups, they don't completely backup every detail of your system.

Keep in mind you can't selectively restore individual apps with a Nandroid backup, whereas you can with TB or MyBackup.

Personally I have MyBackup Root scheduled to make a backup of all my apps at 3am everyday as a precaution in case my system gets corrupted, and I make a Nandroid backup before I flash anything in Clockwork in case the flash doesn't go well and I want to restore everything exactly how it was before the Clockwork flash.
 
Nandroid has become my new best friend. I have three different themes that I like to use. I made nandroid backups of each theme and now can easily go back and forth between the themes.

Keep in mind that info will not go in between your backups. Play a game, send a text message, bookmark a website, your info would not be sved when loading another backup.
 
Can you flash the backup that was created before converting to EXT4 and flashing a rom?

No because that would be RFS filesystem and it will not work once you have converted to EXT4
