Why are my custom tones randomly reverting to defaults?

Ive already done some googling, and people seem to say everytime they connect it to a pc, everytime they reboot, etc. But mine is completely random. i turn my phone off every night and most of the time it remembers, but sometimes it wont. randomly. I havent even noticed it having anything to do with charging or not. it just seems like on random reboots they will wipe out.

the files are there and visible by the phone.... just, reset. why is this??
