Why does bluetooth sound cut out to my headphones but they're still connected?

I have a Galaxy Note 4 running Android 6.0.1. I just bought the Plantronics Backbeat Fit Bluetooth headphones and the sound keeps going out after a few minutes (30 sec to 6min). The headphones still appear to be "connected" and the music player time keeps going as if nothing is wrong, but there is no sound. This happens with any app that plays music or sound. If I push the play/pause button twice, the sound will come back on. If I power off the headphones and power back on, the sound resumes. Changing the volume will not make the sound come back. The problem seems to happen a lot more if there are text messages or other notification sounds happening, but it doesn't happen every time there is a text/notification sound. This problem does not happen with a Galaxy Note 2 or with an iPhone 6. The problem occurs at several locations, including those that would seem to have little interference from other phones. I have spoken at length with Plantronics and they determined the problem is with the phone and they could not help. I have updated the headphones firmware to the latest version.
Music plays perfect through my car and a UE Boom bluetooth speaker.
 
I have a few devices paired to my phone. My pebble, my car stereo, speakers, etc. What I found out worked for me on my Droid Turbo was to remove all the bluetooth devices from my phone and then add them back individually, with the most important one first. If you have other bluetooth devices that are used frequently pair them back after pairing your most used devices and that may solve your issue. It did for me. Good luck!
 
It still does it when the screen is off. Note that it doesn't skip like a scratched CD (where it will start playing by itself again). When the sound stops I have to pause then play in order for the sound to come back.
