Why does my LG G3 D855 Android 5 (Lollipop) keep Re-mounting SD card and Rebooting for invalid SIM?

Been using for a year my LG G3 D855 is running on Lollipop (Android version 5), for the past month I'm encountering following issues;

1.) The Sandisk 16Gb SD card keeps randomly unmounting then re-mounting itself. Whenever i attempt to access anything on the card, it immediately unmounts itself.

2.) Phone runs hotter and it seems the heat is around where the sd card/sim card is mounted. It would say INVALID SIM and reboots on its own.

Can somebody help me? How can I get above 2 issues resolved?
