Why does my OTG USB only work after a factory reset?

I have an HTC Nexus tablet and am very pleased with it. However, although the OTG connection worked fine when I did a factory reset, as soon as I added the apps back, OTG is dead. Rebuilt it twice now and don't want to do it again. Clearly something is blocking it but can figure out whether it's an app or some other config issue. I am wondering if the is a low level way to force OTG support back on, none of the OTG helper apps seem to make any difference.

Should have said, it's a Nexus 9 16GB no SIM.
