Why does the charger connect and disconnect again and again?

Why charger connect and disconnect again and again...

I have a 'Samsung Galaxy Core Prime SM-G360H' when i connect the charger. It starting to connect and disconnect again and again which drop my battery to down. But when I connect it to my Laptop it starting charging frequently. Charger also working on other Samsung mobiles. What can I do to resolve this.
