Why is my battery percentage giving false readings?

I've had my galaxy note 2 for almost a year. I recently started having problems with the phone. It would say 100% and after awhile it would go dead. I plug it up and the battery pops up almost full. Its impossible if it went completely dead. Then this "" keeps popping up on my signal bar and then it cuts off at 67% & reboots .So I have to keep my charger plugged in my phone so it won't cut off sometimes . Can some one give me any answers

Sounds like a dendrite in the battery. See Lithium Battery Dendrite links. As B. Diddy says, you probably need a new battery.
