Why is my mom's Samsung Galaxy Note 4 disconnecting all Bluetooth devices?

My Moms Note 4 has been having strange Bluetooth issues. What has been going on is the Bluetooth will disconnect all Bluetooth devices and turn the Bluetooth on and off, on and off over and over again maybe 20 times in a row then it will stop. If you go under Settings>Bluetooth you can actually see the off/on toggle switching back and forth maybe every half a second. After it does that it, pressing the Bluetooth toggle to "on" it immediately switches to "off."

I have already tried factory resetting the phone. And it has the most recent update. Any idea what is going on?

It doesn't happen all the time. It seems to be random, one second everything will be fine and connected, the next it starts its spasm again. I don't know how to replicate it inside normal more so I don't know how to try it in safe mode.

But does it happen every day, or more than once a day? If so, then see what happens if you use the phone for most of the day in Safe Mode. If the glitches don't happen, I would guess it's a 3rd party app that's causing problems.
