Why is my Nexus 4 rapidly discharging while plugged in?

Nexus 4 rapidly discharging while plugged in

I've around 35% charge when I plug my nexus 4 in, using the original cable. When I check a few hours later, to my surprise the phone simply turned off due to insufficient charge (it is at 1% when I turned it back on). My app that tracks battery level shows that the phone essentially discharged to ~25% 10 minutes after "charging", and to essentially zero in less than an hour. The temperature also goes up to about 50 degrees, versus around 25-30 normally.

The default battery page lists nothing abnormal about which program was the culprit. It's almost like the charge going in the opposite way when I plugged it in! Any idea what happened?
