Why is my phone dying while charging

My phone is an LG X Power, I have had the phone for almost 2 years now and about three days ago I was using the same charger and wall outlet I've always used to charge my phone while in bed, and after about 30 minutes I noticed the battery decreasing, the phone provides a warning of a weak charger, but hasn't provided one for the charger, however it still dies while charging, why? And how can I fix it?

The phone is using more current for the apps than it can draw from the charger. that's usually the sign of a weak battery. After only 2 years, that's usually a sign that you've been letting the battery drop down below 40% (which you should never do).
