Why my phone keep rebooting and only have back button

My phone is HTC desire 816 .. I have problem which is :-
1) My phone keep rebooting (repeating)
2)When it keep display android.phone.com (phone app) , android.process.com(google play store apps) have stoped and etc
3) my IMEI also lost

Anybody can help me to solve this problem .. I really need your help to solve this. .. you can explain the method to me..
