That's from the latest software version (2.11.605.19) that Verizon released back in April/May. Not sure if it's still MR4 or 5, but either way you should be good as that ROM reference says "mr2.x+ or mr3.+ radios".
Google search on the radio baseband (1.48.00.0906w_1) from your screenshot shows that's from the 2.11.605.5 software version released back in Oct. 2011, which is probably MR3 or 4.

this freakin intermittent connection problem made me think maybe my device isn't communicating to the towers properly, though my wife who's not rooted and has newest radio has same problem. Though seems to be a little better now.
