Why isn't my Galaxy S6 active connecting to my AT&T network?

It continuously says "SIM not provisioned MM#2" or something and I don't know what it means. I recently bought the S6 active as a replacement for my s5 active. I set it up and everything was fine. Everything transferred but the phone will not connect to the network. Please help!
