Why isn't my Note 2 able to read the SD card or able to read phone through USB to desktop?

Note 2 doesn't read SD card or able to read phone through USB to desktop

Running android 4.1.2 on an AT&T branded phone, running straight talk.
The SD car is good formatted correctly, but once I got an error sayting I needed to insert my sd card when it was lock in.
I can read the SD card on my desk top if I use a card reader, but can't if i try to read it through usb. I have the phone memory accessible through the phone, but cant read it through usb to desktop. The external SD card cant be read on the phone or though usb, but through a card reader.

any ideas, was working great then all the sudden.
the phone is rooted, running stock 4.1.2 happened 7 months after I rooted it, so not related.

I had this happen once i took the information off the card and then reformatted the card put the stuff back on and worked like a charm
