why does my Galazy Note S4 consistantly disconnect and reconnect from my home WiFi network?

My phone will display "Connected to WiFi Network *****" and the WiFi icon appears, then after a few seconds it disappears. Continuously. When I try and go to a website it says "no connection". The last time this happened I called the cable company, thinking it was my internet connection. It was determined that it was the phone and the next day it was fixed. It's not fixing this time.
