It's the It is One More Clock Widget. It comes with many templates to use.

This is my setup. Only had the phone slightly longer than 24 hours. Not sure if I'm x sticking with Google Now Launcher or going with something else, but I'm liking what I have going and anything without the stock look is out of the question.
