I got a replacement for mine back in April and the second one wasn't any better.
This is one area I hope the improve with the Nexus 5. I stopped using the N4 because of it as it was too unreliable compared to my Z10 (tired of having no reception when indoors at stores, etc.).
I'll give the N4 to the missus when the N5 comes out since she doesn't use her cellphone much so it probably wont bother her.
