The only way on the native ThunderBolt to set your in-call volume is during a call. You can call your voicemail and set it then. Afterward it should maintain that setting.
The TBolt has an issue where sometimes it forgets the in-call volume and the volume automatcially sets itself to max (or min). I have solved this with two programs from Google Play.
One: Volume Locker. . This locks the in-call volume to a default that you set, but allows you to change it duing a call if you need to. Then it reverts back to the default after you hang up. It also locks other volumes on the phone if you like. I only use it for in-call volume becuase that's the only trouble I have with volume.
Two: Volume Control. . This sets the in-call volume to the default that you like without having to be in a call. It also sets other volumes on your phone, if you like.
I recommend both of the above used in conjunction with the ThunderBolt.
