Why does the Galaxy S4 volume change by itself?

Why does the Galaxy S4 volume cha nge by itself

I have gone to settings. All volume is up. But... at times, too many times... all volume shows that it is all the way up yet I can't hear the ring, when I touch the phone, the sound is off. Is anyone else having this issue? Again, the volumes all show that they are up as high as possible.

Are you using any BT devices or anything that might be connecting automatically to your phone? I remember this happening to me a couple times when my BT headphones were around and I'd forgotten to turn them off. (audio comes through the earphones, not the phone)
