Why are my photos no longer saving?

[AC Question]

I have a Samsung Note II and about a month ago my photos stopped saving. Whenever I take a picture or download a picture, I get "Unable to find item" when trying to view it in Gallery or Photos. I know that there is space available and my phone is recognizing the sd card. This is very aggravating and I would be grateful for any help.
