Every Time I swich off my phone and Switch it on again All the sd card data lost!!!

I have Samsung 32GB memory card.I tried all the stuffs to solve this problem but any single tip won't works for me.This problem occurs in each and every phone I tried.Please Help me Some one!!

Perhaps the card is bad, if it does that in all the phones you've tried?
