On outgoing calls I need to hang up and re-dial in order to hear the person I'm calling.

I can see that the call makes it through, I just can't hear it.

When I call the person back they often say "I could hear you, couldn't you hear me?" - of course, no I couldn't.

When I hang-up and redial, it works just fine.

I have a Samsung Note 8 and the Pioneer 2440NEX head unit.

    Any thoughts? Thanks

Being a Bluetooth device I have to ask if you have the setting for Phone audio enabled? Look at your Bluetooth connections, you will have to turn on Bluetooth for this. Then next to each device there is a gear symbol for the settings for that device. Be sure that Phone audio is checked for your Pioneer. I've also seen Pioneer articles about upgrading the firmware, and resetting the unit by unplugging it from the battery for 20 to 30 minutes.
