Are you using Widgetlocker or some other lock screen app that uses the accessibility service? If so, there is a known bug in Sense that does exactly what you are describing. One of the very few annoyances I have with my One.

If you like to get really deep into customization, Zooper is awesome. It will allow you to make some really beautiful custom widgets, but the learing curve is pretty steep. Very cool app though.

Maybe it's something I'm doing wrong, but I wasn't able to get Widgetlocker to work the way I wanted it. I ran into the Accessibility bug hard for the way I use my phone and I have uninstalled it and am just going to wait for 4.2.2.
