Finally got this figured out.... When i create the first widget i drop it on the home screen. The 2nd widget gets dropped ontop of the 1st to create the folder. Those 2 always work. The rest are created and dropped directly into the folder and that was the problem. If i create each widget...

Just wiped the phone again and created the widgets after the phone booted and logged into my gmail account. Same problem.
