I've had the same trouble with my Tab 3 and our kiddie version. Make sure you do all caps for the serial and Wi-Fi address, and make sure you use the Wi-Fi MAC address and not the Bluetooth address like I did :-P it also took a couple of attempts when was doing it right so just keep trying...

Can't help with the phone because I've only had mine for two days, but if you've got the protection plan they'll give you a loaner for the period they have posession of it. OK, it's not your phone, but it's a phone in the meantime
