Okay. I have a google nexus 4 and Cyanogenmod 13 nightly. My phone recently quit working with gapps and SIM. I want to get rid of Cyanogenmod and restore to 5.1.1. Most likely the problem I had, USB port just stops working. Mine miraculously fixed itself by having a cable ripped out forcefully (I do NOT recommend doing this)

When this happened to me I just chucked the phone at the wall while plugged in and it somehow worked. Hope this helps :)

You need to be rooted with a custom recovery, it is called USB OTG. You can get a zip file to install and then you hook it up to a USB OTG Y cable, the USB device and power. Hope this helps :)
