If you have a sprint phone, try uninstalling: --My Sprint --Carrier Call Utility Probably only one of these is the problem, but I don't want to reinstall one to find out which is the problem.

Update 12-2018: Nope, this did not fix the problem! Old review: I solved this problem on my HTC One M8 by uninstalling Android "Device Manager" (by Google). Too bad because I wanted to be able to find my phone remotely which this app allows, but better to keep my permission settings so good...
