sounds like it might either be corrupt (that's fixable) or damaged (physical, not fixable). first, go to the SD Card Association website and download their SD card formatter. Run this off your pc/mac and it'll do a complete format and restore your card's original capacity. If this doesn't work, then most likely your card was damaged (static, excessive heat, moisture/humidity, etc.) and it does need to be replaced.

There is a known issue between the latest Galaxy phones (S3/Note 2) and SanDisk cards manufactured before December 2012, ESPECIALLY 32GB ones. It causes the phone to 'eject' the media on its own and re-mount just as well, causing the media-check every time. It can also lead to actual card damage, so I'd stay clear of your SD card if it's begun to fail.

I am NOT disputing the fix that worked for you, BUT, for me the solution was to reset the phone. Media had suddenly taken over the phone, to the point where I'd lose a couple of percent of battery life every few minutes. There is nothing wrong with my media card (I even had tried a different one, prior to the reset). I did the reset at least (guessing) six weeks ago and I have no more battery drain. Similar problems don't always have the same solutions...glad yours is working.
