I agree with mustang7757, it's likely related to some app you installed. "Phone cleaner" apps are most likely, but other apps can sneak this kind of junk in as well (like file explorers, battery savers, gallery apps, etc.). You may have to uninstall apps one by one until the problem goes away.

I really screwed up.I disabled the wrong App. How can I enable it?

Lololol.

The app in that screenshot isn't disabled, and can't be disabled.

Regardless the process is go to settings, apps and find the app you disabled, tap it and tap the enable button.

I lost all of my home screen icon set up.Also I don't see enable?

Unless you are seeing a command line interface on your screen, you didn't disable the UI app. Now, a lot of the stock launchers do crazy things, like delete everything, and rarely is there anything you can do to recover it. Best you can do is set it up again, or use a different 3rd party launcher like Nova, set it up, and then Nova can backup the setup.

FYI, when you see a greyed-out Disable button, that means the app can't be disabled -- it's always enabled.
