I have had Android phones for many years now, currently OnePlus 8T. I always only use the speakerphone and wonder why, with all the multitude of features and settings, they do not allow you to keep the speakerphone always-on as default? I think it should be an option in the System. Yet they don't even allow apps for this.
My other huge beef is why you cannot clone a phone when you get a new one. You can copy all the apps but then always have to spend 10 hours adjusting all the settings in the apps and phone system. It seems like we should be able to just clone everything when you buy a new phone. Why don't they do this?
 
Welcome to Android Central! There are so many possible use case scenarios that it's not feasible to include all of them in any device's stock settings. I would encourage you to send feedback to Google and/or OnePlus to consider adding that feature. In the meantime, you can probably use a 3rd party app like Automate by Llamalab: https://llamalab.com/automate/doc/block/speakerphone_set_state.html

Some manufacturers have their own "phone clone" solution (like Huawei: https://consumer.huawei.com/en/emui/clone/). Google's built-in transfer utility when setting up a new phone actually works pretty well, as long as you use the direct USB connection method (I think the wireless restore from Google Drive isn't as complete). But there will always be some things that don't transfer over as a consequence of security, like logins (especially to sensitive accounts like banks), credit cards, authenticator tokens, etc.

One plus does have its own clone cloud service along with Google back up

As for calls being defaulted to speaker not that I know of

If I answer my Flip 4 while it's closed, it defaults to speakerphone... but you probably don't want to upgrade to a new phone just for this lol.

I looked through the accessibility settings and surprisingly there's nothing there... I would gues it is to protect us from obnoxious people. Public places are bad enough already.
