Some users have reported a potentially fatal camera fault with the device.

The camera would work fine for the few days, and then, would suddenly stop working, showing a "Warning: Camera failed" message.

A reboot would not fix it. Heck, some even reported that even a factory wipe and reset would not fix it, meaning that the only solution is a replacement.

Not sure if this is SW or HW related, but I hope this is fixed soon.

Note that your GS5 may be affected. So don't be alarmed if this occurs.
 
I got the "Warning - Camera failed" message, but then I realized I had a "flashlight app" toggled on. Turned it off, then the camera was fine.
 
I had this warning the other day, but I was able to reboot my phone, and it's working fine now.
