After restart, phone seems to forget the SD card and all the data on it

I have a Moto G6 Play with Android 8 and a SanDisk Ultra PLUS 128GB microSDXC card installed. I installed the card in my phone and formatted it as portable storage and didn't have any issues until I restarted my phone. After reboot, the phone seemed to forgot about the card, and any apps that had data on the card - Audible, podcast apps, music apps, Google photos - couldn't access the stored data and I couldn't download any new data while the card was set as the storage location. I had to take the card out, copy the data to my PC, reformat it again, set it up as portable storage and copy the data back. After this happened a second time, I decided not to restart my phone again if I could help it, but today I was prompted to install a security update and I have the same problem.
I've seen variations of this in other posts, but no solutions that work. Does anyone have any suggestions? I'd rather not keep going through this process after a restart.
I've already replaced the card a couple of times with similar issues, and I don't think it's a phone problem, but I could be wrong. Thanks for the help

Welcome to Android Central! No one answered this from 5 years ago -- posts can get lost in the shuffle sometimes, especially if they're not answered within the first few days.

The first thing to consider would be if the card might be counterfeit, which is unfortunately all too common these days. See this article for how to test: How to Spot a Fake MicroSD Card and Avoid Being Scammed
