I have been having a similar problem since updating the phones firmware to I9300XXEMC2. I have a couple of alarms set in the morning each day. The first alarm always goes off for 1 second and then stops. The second alarm an hour later seems to work fine. I have tried deleting the alarms and creating new ones but it doesn't help. I also tried clearing the clock data from the application manager in phone settings.

Have you found a solution?

I am going to try a factory reset and see if that solves it.

Ben

Are there any other alerts or notifications on your phone at that time? It may be that a second notification is terminating the first one.

ozgurs,
What's different between the occasions when it works and when it doesn't?
Is the phone normally face down, or in a dock, or what?
Do you make any motions on those instances when it doesn't work (move your hand near it, etc.)?
Is it a 3rd party app? If so, which one?
Did this behavior just start recently?
Do you have any sound profile apps installed?

After a factory reset my alarm went of correctly this morning. Hopefully that has solved the problem.

BigBenO,
Keep us posted on whether the problem comes back or not.
It's actually a good idea to perform a Factory Reset following an OTA, but most people prefer not to do that (for obvious reasons).

I got this problem too with my Galaxy S3. There was no direct fix, instead I had to download a third party app, "AlarmDroid". I haven't had a problem since.
