App deleted from App list, but not Application Manager

My toddler got a hold of my phone and deleted my "phone" icon/app from my App pages and Home Page. It still appears in the APPLICATION MANAGER, but I cannot figure out how to get it back onto my home screen or the app list.
Having to go into Contacts, phone, then do whatever I need is getting to be a huge PAIN.

Hopefully someone has advice.

Is it still in the app drawer?

No It's gone from the Home Page and App drawer (which I called App list), but it still shows in the Application Manager.

I think you may have to do a reset.
