App that connects to Bluetooth LE based vital sign monitors?

Hi,
+ Looking for an app that connects to Bluetooth LE based vital signs monitors such as
* Heart Monitor / Blood Pressure Monitors
* Blood Sugar Monitor
* Pulse Oximeter
* Weighing Scale

+ And, that allows me to send the data to my server

I am hoping/sure that somebody has already tried this? (any opensource android applications?)

The device manufacturer usually has software available for it. But to read the device and send the results to your server? That requires a custom program.
